Applications
Referrals can be received from any source although they generally come from health and social care professionals, particularly psychiatrists, CPNs and social workers.
There is a comprehensive 6-week assessment process for residential referrals to ensure that the demanding therapeutic environment is going to be right for the individual. For day service referrals, the assessment is usually carried out over a full day.
Pele Tower Service remains in close liaison with the referring agencies and any other professionals involved with the service-user throughout their treatment. Pele Tower staff will attend MDT meetings, CPAs and other reviews as necessary.
Service users are responsible for their own medication.
The following forms must be completed and returned to Pele Tower
before assessment can be initiated:

Download Standard Funding Contract (for Day Service)
Download Standard Funding Contract (for 2-year placement)
Applicants must complete the Application Form, Release of Information Form and the relevant Standard Funding Contract Form or assessment cannot be initiated.
